If you do not have dental insurance and are interested in taking good care of your teeth on regular bases for a fraction of the cost, we can help you.
In-House Dental Plan Options
For our patients without insurance, we are excited to offer the following In-house Dental Plans. We are pleased to offer an in-house membership plan to help our patients maintain regular preventative care and procedures. In exchange for an affordable annual enrollment fee, you’ll receive preventative maintenance and discounts on many commonly performed dental procedures.
Adult Plans
Basic Plan

- Comprehensive exam (once)
- Needed x-rays determined by the dentist*
- 10-15% off all additional dental services depending on method of payment (excluding financing)
- 20% off CBCT/3D Imaging
- Exclusive access to promos and specials

Adult Plan
(12 years and up)
Includes
- Adult prophylaxis/cleaning (2 per year)
- Doctor exams (2 per year)
- Needed x-rays determined by the dentist*
- Limited/emergency exam (2 per year)
- 10% off all additional dental services*
- 15% off all additional dental services if paid in cash.
- Exclusive access to promos and specials
- Fluoride varnish (2 per year)
(in the absence of gum disease)**
Adult Perio Plan
(18 years and up)
(Perio maintenance is for patients who undergo initial Scaling and Root Planning (deep cleaning) of all 4 quadrants. SRP IS not 100% covered procedure under PERIO PLAN, however with PERIO PLAN you get 25% off of all needed SRPs)**
Includes
- Everything from Adult Plan
- Periodontal maintenance cleanings (4 per year total)
- 25% OFF SRP procedure (deep cleaning)
- If SRP completed, plan extended by 1 month (13 months) to get all 4 cleanings after initial deep cleaning.
**The choice of the Adult Plan vs. the Adult Perio Plan can only be determined by assessment of patient gum health by the dentist. If patients enroll in the Adult Plan prior to your first visit and the dentist determines the Perio Plan is necessary for the patient’s gum health, the patient will be responsible to pay the difference of $200 between Adult and Adult Perio membership levels.

Other Deluxe In-House Membership Plan Terms & Limitations
Benefit coverage is effective for participants who remain enrolled in their plan at least until the completion of treatment. If enrollment expires before treatment is completed, coverage is no longer available.
There’s no ID card, no group or member number to bring. All your membership information will be kept in your electronic record. Your effective date is the day you sign up and your renewal date is the same date every year.

- This is a dental discount plan and is NOT dental insurance.
- It is good only Deluxe Dentistry, LLC. Therefore, if you are referred to a specialist, they will NOT offer this discount.
- Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, care, disability or workman’s comp type insurances are involved, this discounted plan cannot be used.
- This plan is NONE Transferable-Family members cannot be substituted in for another family member.
- It is NONE Refundable-no refunds given if patient chooses not to use their dental plan.
- Rates are subject to change annually.
- Payments for services are due at time of service.
- If you choose to extend your payment for treatment by paying through CareCredit® (or another financing option), the discount on all treatment offered in the practice is reduced to 0%.
- Adult vs Perio plan can only be determined by your dentist based on your gum health.
- If you enroll in Adult Plan prior to first visit and dentist determines that you need to be in Perio Plan-you are responsible to pay the difference of $200.
